1. General
1.1 Salt Lake City offers a voluntary fitness program for its regular full-time and regular part-time employees and their eligible family members.
1.2 The City will pay a portion of the cost of a membership in a fitness facility for participants in the employee fitness program. Membership costs are reimbursed using the employee’s accrued vacation time up to the maximum amount determined by Administrative Services and approved by the Mayor or the Mayor’s designee. The maximum reimbursement amount is $500 for regular full-time employees and $250 for regular part-time employees on an annual basis.
1.3 Departments which provide fitness facilities for their employees may choose not to pay for other fitness facility membership costs.
1.4 The use of vacation pay for payment of membership costs at a fitness facility reduces the participant’s accumulated vacation leave.
1.5 Definitions:
A. Participants: Regular full-time or regular part-time employees, their spouse or adult designee and/or their dependent children who live in the same domicile; who successfully complete their initial probationary periods; who have adequate accrued vacation time; and who agree in writing to meet the requirements of the program.
